When the antonym is not prefix-formed (like मित्र-शत्रु or सुख-दुःख), use the "axis check." Ask: what is the core dimension this word lives on? मित्र lives on the friendship-enmity axis. सुख lives on the pleasure-pain axis. Now scan the options for the word on the opposite end of that axis.

Picking the most dramatic-sounding opposite instead of the true antonym. For 'मित्र', some candidates pick 'सहायक' thinking "the opposite of a friend is someone unhelpful" — but सहायक (helper) is on the ally-end of the axis, not the enemy-end. शत्रु (enemy) is the only correct antonym.
Treating domain-adjacent words as synonyms. In the 'समुद्र' question, some candidates pick नदी (river) thinking it is water-related. A synonym must be the exact same thing with a different name — not a related thing. नदी is water, but it is not the ocean.

Confusing विलोम with विपरीतार्थी in passage questions. When the question says "निम्नलिखित में से कौन-सा शब्द 'X' का विपरीतार्थी है," it is asking for the antonym, not a contextual opposite. Apply the same axis-check methodology.
Not verifying that the chosen synonym actually substitutes in a sentence. For hard पर्यायवाची questions with unfamiliar words, do a quick mental substitution: "Can I replace the root word with this option in a simple sentence and retain the meaning?" If yes, it is a valid synonym. This catches false synonyms — words that are in the same semantic field but not interchangeable.
